8) Fax your forms to our office.
All of the packages above include 3 button vests. If you'd like to add a 5 button vest it would be an additional $5.00 to your order. The price also includes the cost of shipping one way to your location. When you are finished and ready to return you can ship using any method you choose or we will include a return label for you to attach to the box for an additional $12.00 per outfit.
